Athletic Physicals will be provided for CHS and CMS athletes on Friday, May 24th beginning at 9am. The physicals are being provided this year by East Geogia Healthcare Center free of charge. In order to see a healthcare provider, the front page of the "GHSA Physical Form" must be completed and signed by a parent/guardian.
